1) Hover over the Electronic Participation Forms tab and select the Registration Form.

2) Enter the First Name, Last Name, and Student ID of the student you are completing forms for

3) Please complete all fields on all forms. To sign, hold down the button on your mouse and move the mouse to create a signature.

4) Repeat the process of filling out and submitting all Four forms listed under the Electronic Participation Forms

5) Once all Electronic Participation Forms are complete, hover over the Download and Print tab and select the Pre Participation Examination.

6) Print the Pre Participation Examination form and take this with you to your Physical
